NHS Risk Registers & Board Assurance
Risk registers and BAFs that boards actually use.
Overview
Too many risk registers are graveyards of stale entries and amber-rated ambiguity. We rebuild risk registers and Board Assurance Frameworks from the ground up, connecting strategic objectives to the risks that threaten them and the assurance that proves control.
The result is a living system: directors own their risks, gaps in assurance are visible at a glance, and the board's attention lands where it matters most.
What we deliver
- Risk register redesign and quality review
- Board Assurance Framework development
- Assurance mapping against strategic objectives
- Risk maturity assessment and improvement roadmap
- Divisional, corporate and board-level risk training
The outcome for you
A BAF that drives agenda, not just compliance
Visible assurance gaps before auditors find them
Directors who genuinely own their risk profile

Frequently asked questions
Our risk register has hundreds of entries. Where do we start?
With a quality review: we score every entry for clarity, ownership, scoring honesty and current controls, then work with risk owners to consolidate, escalate or close. Most registers can be cut dramatically while becoming twice as useful.
What is the difference between a risk register and a Board Assurance Framework?
The register captures operational and clinical risks in detail; the BAF shows the board the small number of strategic risks to its objectives, the controls in place and the evidence those controls work. We build the two to connect, so escalation is visible rather than manual.
How often should the board review its principal risks?
Formally at every meeting through the BAF, and in depth at least quarterly. The question that matters is not the score but the movement: what changed, why, and what the board is being asked to do about it.
