Clinical Risk Management & Patient Safety

Embedding proactive safety culture across every clinical pathway.

Overview

Safe care is not the absence of incidents; it is the presence of systems that anticipate, absorb and learn from risk. We work alongside clinical and operational teams to identify, assess and control risk at the point of care, translating national policy into practice that works on a busy ward, in a theatre suite or across a community caseload.

Our consultants are fluent in the NHS Patient Safety Strategy and the Patient Safety Incident Response Framework (PSIRF). Whether you are transitioning from the Serious Incident Framework, rebuilding a stalled safety programme or preparing evidence for regulators, we bring the structure, challenge and clinical credibility to move you forward.

Frequently asked questions

What is PSIRF and does it apply to our organisation?

The Patient Safety Incident Response Framework is the NHS approach to learning from patient safety incidents. It applies to NHS trusts and is expected of any provider delivering NHS-funded care. Instead of investigating every serious incident in the same way, it asks organisations to plan their response around where the most learning lies. We help providers write that plan, train their teams and embed it into everyday practice.

Can you work alongside our existing quality and safety team?

Yes — that is how we prefer to work. Our consultants slot into your governance structure, add senior capacity and specialist skills, and deliberately transfer capability so your own team is stronger when we leave.

How do you measure whether patient safety work is working?

We agree measures with you at the outset — typically a mix of incident themes, repeat-event rates, audit results and staff safety-culture feedback — and report against them honestly, including when progress is slower than planned.
